These forms are all also available in Physical Sciences Center F-Wing (PSF), room 470.

Department Consent Form (below)
Use this form to request a suppressed upper division line number such as PHY 592. Please have the form signed by the professor who will be giving you a grade at the end of the semester and your advisor, if they are not the same person. Bring the signed form to PSF 470 to receive the suppressed line number.

Supervisory Committee Nomination Form (below)
Once you are ready to form your supervisory committee, you will need to fill out this form. Please select four faculty members if you are in the doctoral program, or two if you are in the master’s program. Make sure it is acceptable to them to be on your committee. After you have made your selection, return the form to the graduate coordinator, who will inform Professor John Venables, lead graduate advisor, of your choices. He will then give you the names of two faculty members that you will need to choose from to be your final committee member. After you make your decision and speak with your chosen candidate to verify their availability, please inform the graduate coordinator of your final choice.

Petition to the Graduate College [1]
Use this form if you are requesting to take more than 12 credit hours a semester, if you are requesting to work more than 50 percent time (summer only), or if you need permission from the Graduate College while earning your degree. Only your committee chair and the chair of the department need to sign this form. If you do not have a committee chair, have your initial advisor sign for you.

Graduate Supervisory Committee Change Form [1]
If you are replacing a member of your supervisory committee, all of the remaining members and the new member will need to sign this form, as well as the department chair. The replaced member does not need to sign.
